Our Permanent Supportive Housing apartment complexes provide safe, stable, and affordable homes for individuals. Each resident receives access to tailored services, including case management. By addressing both housing and support needs, we create opportunities for our residents to thrive and build a brighter future.
Enhanced Case Management
Our experienced case managers work closely with clients, developing personalized plans to address unique challenges and guide them towards sustainable solutions.
Housing Navigation & Tenancy Support
Navigating the complex housing landscape can be challenging. Our team is here to guide you through the process, ensuring you find the housing option that best suits your needs. We assist in securing tenancy, supporting clients in understanding their rights and responsibilities as tenants, and fostering positive landlord-tenant relationships.
Supportive Outreach
We offer transportation services to help with daily needs and appointments, wellness programs to promote healthy living, community events to foster connection among residents, and many more resources to help develop and maintain life skills.
Substance Abuse Treatment
We are proud to offer a program designed to help clients with substance abuse challenges. We help individuals break free from the chains of addiction and lead healthier, more fulfilling lives. Asking for help takes tremendous courage, and we're here to help on every step of the way.
Homemaker Caregiver Program
Through our partnership with Partnership HealthPlan, we are excited to offer the Homemaker Caregiver Program to assist with daily tasks, helping you live comfortably and confidently in your own home. Whether you need a little help around the house or assistance with personal care, our caregivers are here for you. We offer Household Assistance, Personal Care, Enhanced Health Support, and Aging in Place services. Our goal is to help you stay independent and thrive in your home.
